living better

Living Better is a network of organisations across West Somerset working closely together to support the people of our local community with their health and wellbeing.

Living Better was established in 2017 and over that time we have worked closely together on a number of projects and health and wellbeing initiatives to enable patients to develop the knowledge, skills and confidence to reach their own health and wellbeing goals.

NEW MATERNITY SELF-BOOKING SYSTEM AT T&S

somerset badgernotes is an NHS service providing a single point of access for all maternity referrals in the Somerset area. The aim is to make sure you have access to the right care from day one. they will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away.

If you are more than 6 weeks pregnant you can use this maternity referral form to tell us about your pregnancy.  Your pregnancy care will be provided by a team of midwives based on which GP surgery you are registered with. Once you have submitted your self-referral you should receive a confirmation e-mail to let you know it has been received.

You can also use this to refer a pregnancy on behalf of a woman.

Getting help and support around the menopause

If you, or someone you know, is experiencing the menopause and wants help with managing your/their symptoms and to understand how you/they may be affected, please watch this helpful webinar developed by Dr Kathryn Patrick a local Somerset GP recorded in March 2023. The webinar is also available in sign language for those who are hard of hearing – this was recorded in October 2022.

For more details of the webinars and of the overall Somerset NHS Menopause Service please see Somerset NHS Menopause Service – NHS Somerset

There is a list of useful resources on the NHS Somerset Menopause Service web page that talks about the pilot menopause service running in Somerset.

If you would like to speak to your primary care team here at your practice about HRT, it will speed things up if you let us know you have watched the webinar and also include your Body Mass Index (BMI) which can be calculated at BMI calculator | Check your BMI – NHS | Please fill in your details (www.nhs.uk), and Blood Pressure (BP) if possible.

Please also complete and include the following menopause symptom questionnaire – Menopause Symptoms Questionnaire – The Menopause Charity.

Think Travel – Somerset

A new one-stop travel information website has been launched by Somerset County Council to support more local people to access public and community transport.

If you don’t own a car – or you want to leave it at home and travel more sustainably, Think Travel can help you with your transport options. Whether it’s getting to the doctors, shops, travelling to work, college, or school, or just into town to meet friends or family, the new travel planner shows you what’s available. It covers public transport, community transport, the Slinky demand responsive transport service and car sharing.

To start your search, all you need to do is enter your journey start point and destination in the planner and Think Travel will find the available options for you. As well as information on train and bus services, there’s also a dedicated car sharing notice board you can sign up to, where offers or requests for car sharing journeys can be posted. The aim is to grow this facility and encourage local car sharing networks to evolve.
