REGISTRATION

You can now register online here

This form should take 10 minutes to complete and it will take about 2 working days to get you registered. If at any point you find this form too hard, you can Use the nhs service below, print the two registration forms from the links below or collect paper registration forms from our surgery reception.

You do not need proof of address or immigration status, ID or an NHS number.

registration packs are available from our surgery reception. 

to register you will need to complete a GMS1 registration form and a medical questionnaire.

Both forms need to be printed, completed and then brought, posted or emailed to the surgery.

If you take regular medication please make an appointment to meet with our primary care pharmacist to enable them to discuss your health needs and review your medication. Once this has been done you are able to order your repeat medication either online or via our prescription telephone line 01643 703441 (Option 2).

please highlight on the forms or let of member of our reception team know if you have any information or communication needs, and how we can help to meet those needs.

This can then be recorded and flagged on your record so any member of the surgery team that needs to access your record will be informed. We can also, with your permission, share this information with other parts of the NHS – e.g. hospital/consultant referrals.
